Dependent Tuition Waiver Form - An employee's dependant is one who satisfies the general eligibility requirements for purposes of eligibility for health benefits under Mesa State College health insurance.
Tuition Waiver Form - The tuition waiver program is designed to assist employees in expanding their education through courses at Mesa State College.
Payroll Direct Deposit Form - This will allow the college to directly deposit your bi-monthly or monthly pay check into a designated checking or saving account. You will need to return this form in person with a copy of a cancelled check or saving deposit slip containing your information.
Address Change - This form will update your current address for the departments selected on the form. You can also change your
address on-line through your MavZone account.

Third Party Billing - Some employers/businesses pay for tuition, fees and books for students. This needs to be set up in advance with our A/R Office. The authorization can be saved and e-mailed or printed and faxed to us at 970.248.1136.

College Opportunity Fund - The College Opportunity Fund (COF), created by the Colorado Legislature, provides a stipend to eligible under-graduate students. The stipend pays a portion of your total in-state tuition when you attend a Colorado public institution or a participating private institution.
Health Insurance Application - Health insurance options are available to students attending Mesa State College. Please review the enclosed information and application form found in the above link. Completed forms need to be turned into the Business office with payment.
