Practical Nurse

begins once a year in the fall semester

APPLICATION PROCESS

TO APPLY:

1. If not currently a Mesa State student, submit a completed Mesa State College application, including application fee, through the Admissions Office or reactivate application file if you have missed one semester at Mesa State College, declaring Practical Nurse as area of emphasis.

2. Submit copy(ies) of all transcripts from other colleges/universities to MSC Office of the Registrar, (contact previously-attended schools and have them send transcripts to MSC Registrar - who will then do a transcript evaluation and make the decision on which courses taken at a previous college will transfer to Mesa State College.) Transcript evaluation by Registrar is required for all courses taken at other colleges. Applicants must have at least a 2.0 ("C") on a 4.0 scale for all courses required for completion of the Associate of Applied Science. This policy applies regardless of when the course was taken.

3. Complete (or be about to complete) general education requirements and prerequisites. If currently enrolled in a prerequisite, include an instructor-signed "request for grade" form verifying the grade you are expected to receive.

4. Submit a separate PN Application prior to March 1 for fall entrance.

5. Applicants must take the "TEAS Test" (Test of Essential Academic Skills). The required test measures an applicants basic skills in reading, math, science and English. The cost to the applicant is $20.00. Payment for the TEAS test must be made in advance at the Accounting/Business Office at Mesa State College in Lowell Heiney Hall. They will accept cash, checks, Visa, Mastercard, and Discover cards. You must take a copy of your receipt with you to your TEAS test session and give it to the proctor.

Application points earned for TEAS Test Score:
65 - 69 = .5 point
70 -79 = 1 point
80 -89 = 2 points
90 - 100 = 4 points

Teas tests will be scheduled approximately a month before the application deadline. Once scheduled, there will be a link here that you will use to schedule your TEAS test.

Out-Of-Town Applicants: If you are an out-of-town applicant who cannot make the posted testing sessions, please contact ATI Testing at 800-667-7531and ask about TEAS test proctoring in your area - and be sure to tell ATI that you need to take the TEAS Test for Mesa State College (in Grand Junction, Colorado) for the PN program.

6. Submit a copy of appropriate CPR certification (CPR/AED for Professional Caregiver (ARC) or CPR for the Healthcare Provider (AHA).


Items 1 through 6 must be completed prior to March 1 for fall entrance. Remember, it is the student's responsibility to make sure the nursing office has these items in your file by the deadline!


UPON ADMISSION TO PN PROGRAM:

7. Pass a criminal background investigation through American Databank at http://www.mesacx.com. The cost for the student is $60. Student must fax a copy of "Disclosure and Release Statement" to American Databank (form is on American Databank website) and also turn in a copy to Kelly Losher in the nursing department.

8.Submit a Physical Form. Physical form must be completed with attachments and turned in on the first day of class. The first side must be filled out by the student and the second side must be filled out and signed by your health care provider. Proof of immunization is also necessary including Measles/ Mumps/ Rubella, Tetanus, Hepatitis B (this is a series and you will want to begin this soon), as well as a current TB skin test (must be within one year). Though it is currently not required we also recommend the chickenpox (varicella) vaccine for the student as well. You MUST ATTACH proof of your vaccinations to the physical form.

(Any student receiving a varicella vaccination should do so at least one month before beginning nursing classes. Students should not attend clinical rotations for 30 days after a varicella vaccination due to the rare possibility of on contracting varicella after the immunization and exposing patients and faculty at clinicals.)

9. Submit proof of professional liability insurance to the nursing office prior to being allowed entrance into a clinical facility. If you wish you may apply for insurance through the National Professional Group as a student nurse. The website is http://www.nso.com. Should you choose to go through a different company, the policy must be comprehensive general liability insurance covering bodily injury and property damage liability with minimum coverage limits of $1,000,000 per occurrence/$2,000,000 general total limit, and medical professional liability insurance with minimum coverage limits of $1,000,000 per claim/$3,000,000 annual aggregate.

10. Activate your MSC e-mail account by going to the Tomlinson Library computer room (if not already activated). This account is free for MSC students, and all nursing students are required to activate - and regularly check Mesa State e-mail.

11. If interested in participating in the AmeriCorps UCAN Scholarship Program, you must attend orientation at a time and location to be announced. Failure to attend an orientation program will eliminate you from consideration for the program for this semester. For information on what the AmeriCorps UCAN scholarship program is, visit the website www.americorps.gov, or call 1-888-507-5962 or e-mail edawardvoucher@cns.gov.

12. Call the Mesa County Workforce Center at 248-0871 one month before classes begin regarding possible scholarship dollars. Talk to your employment counselor if you have one, or talk to Mark Fugere (248-0867). You must sign up one month before classes begin.

13. Purchase name tags; you must have a name tag to do clinicals at area hospitals. See Kelly Losher, Administrative Assistant for the PN program, located at the WCCC/Bishop Campus, 2508 Blichmann Avenue, Building B, room 145, phone number 255-2630. You must fill out a form, pay cash for your name tag ($5.00 for a pin tag, $7.00 for a magnet tag), and it will be ordered for you and given to you on the first day of class. Again, you must pay in cash.

Items 7 through 13 must be completed and turned in by the first day of class. It is the student's responsibility to see that these items are
turned in by the deadline!

 

AFTER CLASSES BEGIN:

Read PN Student Handbook, sign form confirming handbook was read and turn in to PN Administrative Assistant.

Sign Academic Honesty Agreement stating that you will in no way plagiarize, cheat, or knowingly provide false information in the PN program and turn in to Administrative Assistant.
